By checking this box, you confirm and agree that your participation in this program is voluntary and that all requirements must be fulfilled to qualify for the bonus payout. You also understand that all qualifications must be met by both the referrer AND the person being referred to qualify. If any step is not completed, the referral bonus will not be paid out.

EMPLOYEE QUALIFICATIONS:

Applicant must:

Be a new applicant within the last year, meaning has not applied or worked with ISS for at least one year;
Apply within 30 days of this form being submitted.
Meet all hiring requirements and be offered, accept and start a position with ISS;
Indicate the referrer's name as the referral source during the interview; and,
work at certain amount of hours before payout is processed.
